[President] Koji Fukuoka, Ph.D, Master Mariner.

Koji Fukuoka is the developer of RMQMPTM (Risk Management and Quality Management Process approach) model, an integrated framework for accident prevention, safety design, and risk management. He is a scientist-practitioner specializing in safety management systems (SMS), accident investigation, and systematic accident prevention.

He earned his PhD in Maritime Science and Technology from the Graduate School of Maritime Sciences, Kobe University (2016). Prior to his academic career, he served for over 20 years in the Japan Coast Guard, where he was engaged in maritime safety, counter-terrorism, and international investigations on illegal migration.

He later worked as an accident investigator at the Japan Transport Safety Board, conducting numerous accident investigations and producing official reports. During this period, he also studied accident investigation at Cranfield University (UK).

At the international level, he contributed to the International Maritime Organization (IMO) as a Correspondence Group Analyst for the III Sub-Committee and served as an IMO consultant, providing training on “National Training Course on Marine Casualty and Incident Investigation” to government officials in the Philippines.

He subsequently led the development and implementation of a Safety Management System (SMS) at the Okinawa Institute of Science and Technology Graduate University (OIST), where he also established a university-wide risk management framework. As a professor at Kyushu University, he conducted research and education on accident prevention and safety science.

Based on these multidisciplinary experiences across field operations, accident investigation, international regulation, and academia, he developed RMQMPTM model as a unified theoretical framework that addresses the limitations of conventional accident models.

In 2025, he published “Accident Prevention and Investigation: A Systematic Guide for Professionals, Educators, Researchers, and Students” (Wiley), presenting a comprehensive theory of accident prevention and safety design. His current work focuses on the global dissemination and implementation of RMQMPTM model across industries and educational institutions.

Accident Prevention and Investigation: A Systematic Guide for Professionals, Educators, Researchers, and Students (Wiley, 250 pages, published in  2025): The book is designed to enable university faculty, staff, and students to conduct accident investigations and analyses and to proactively prevent accidents at universities.
The RMQMP(TM)model (Revised 2025) was officially published.

Safer Seas: Systematic Accident Prevention (CRC Press Taylor & Francis Group publication, 268 pages, published 2019). The book covers accident prevention and recurrence prevention in the maritime industry, accident investigation and analysis methods, SMS, etc.

His major scientific publications include “A New Approach for Explosion Accident Prevention in Chemical Research Laboratories at Universities,” Scientific Reports , published in 2022. The paper identifies problems with university accident prevention measures and provides new tools and problem-solving approaches to prevent accidents from occurring in university laboratories.

Doctoral Dissertation (Kobe University, 2016) “Model of marine accidents developed by the Swiss cheese model and recommendations for systematic accident prevention. In this dissertation, the Swiss cheese model is applied to actual marine accidents to resolve the unresolved issues of the model, clarify accident patterns, and develop a new accident model (later referred to as the RMQMP(TM)model[2016 version] ), which uses algorithms to show the relationship between accidents and accident contributing factors.

“Relationship between latent conditions and characteristics of holes in marine accidents based on the Swiss cheese model”, WMU Journal of Maritime Affairs、 2016. We analyzed 84 serious marine accidents using a model that integrates the Swiss cheese model (SCM) and the SHEL model, and identified six patterns of accident occurrence.

“Visualization of a hole and accident preventive measures based on the Swiss cheese model developed by risk management and process approach”, WMU Journal of Maritime Affairs, 2016. The holes in the Swiss cheese model were defined as unacceptable regions in the ALARP. Four marine accidents were investigated and analyzed using SCM theory to clarify the characteristics of the holes.

“Accident models and their applicable industries“, The Journal of Science of Labour, 2017. This article introduces the historical background and classification of accident models, along with accident models that can be applied to various industries.(Japanese)

“Convergence of the epidemiological and systemic accident models”, Journal of maritime researches, 2017. A ship sinking accident that occurred in the UK was analyzed using the RMQMP(TM)model and the STAMP model. The RMQMP(TM)model was developed from the Swiss Cheese Model. It has been shown to converge the epidemiological and systemic accident models.
