30-minute lecture on “Utilizing Near Misses and Accident Prevention” was given to 176 industry and academia people including students, as part of the Lecture Series held by the School of Interdisciplinary Science and Innovation, Kyushu University on June 9, 2025.
Detailed explanation of the turning points (factors) that divide events between accidents and near misses was made based on actual accidents that occurred in the maritime and aviation fields. I showed how the intervention of a third party who learned lessons from actual accidents contributed to a near miss in which all three people were saved, whereas they would have normally been killed without the third party. On the other hand, in an accident, two people were killed when rescuing their colleagues. Lack of practical training for emergency case led to neglect of safety measures, and group psychology, which made them feel they had to help their colleagues, also played a role.
Analysis of many accidents has revealed that near misses occurred before the accidents happened. I introduced a tool for preventing accidents by comprehensively and scientifically analyzing near misses that occur during scientific diving, and explained that the tool can also be used to identify safety issues and improve safety in laboratories and universities even when near misses do not occur.
